Use this when families need one clean note on required forms, waivers, signatures, deadlines, and where to turn things in before the missing-paperwork problem starts showing up at practice, on meet week, or right before a trip.
The forms note should keep families from asking basic logistics questions one by one.
List the specific forms, waivers, or signatures instead of saying “finish your paperwork.”
Name the due date and what happens if it is late.
Say whether it should be uploaded, emailed, or physically turned in, and to whom.
